If you are uninsured or not using insurance (self-pay), you are entitled to a Good Faith Estimate (GFE) of expected charges for therapy services under the No Surprises Act. This estimate helps you understand the cost of your care before we begin.

• What to Expect: I provide a personalized GFE to every self-pay client before their first session. The GFE includes planned services (e.g., therapy sessions) and their estimated costs.

• Requesting a GFE: If you need a GFE at any other time, contact me at zoe@zoerosetherapy.com or (847)260-8722. I’ll provide the estimate within 1-3 business days.

• Your Rights: The GFE is an estimate based on the information available at the time. If your final bill is substantially in excess of the expected charges, you have the right to dispute the charges through the federal dispute resolution process.
